Local Wellness Policy Triennial Assessment

All School Food Authorities (SFA’s) that operate the National School Lunch Program (NSLP) are required to complete another Local Wellness Policy Triennial Assessment by June 30, 2026, even if the 2023 assessment was submitted after June 30, 2023.

The triennial assessment is comprised of four steps:

  1. Measure the extent the local wellness policy complies with the USDA wellness policy requirements and model policy language;
  2. Demonstrate progress made in reaching the goals of the wellness policy and measure compliance with the wellness policy for your school/each school within your district/residential child care institution (RCCI);
  3. Make the Triennial Assessment results available to the public on the SFA website; and
  4. Notify NDE of completion and the methods used to meet the public notification requirement.

Upload completed Triennial Assessment documents into the Attachment List in the SY2025-2026 School Nutrition Programs Application Packet in CNP. This will help ensure that the documents are retained and are accessible for both your staff and the NDE program specialists who will review this documentation during your SFA’s next Administrative Review. Important Note: CNP access is limited to specific roles, such as the Authorized Representative, School Nutrition Director/Manager, or Claim Contact. NDE strongly recommends that one of these individuals, preferably someone who already has active CNP login credentials, upload the triennial assessment documentation.  All documentation is submitted in the same designated location within the CNP system regardless of which user completes the task.

The Google Doc resources are set to force a copy. After clicking the resource link, please click on the blue “Make a copy” button; the template can then be edited. You may provide access to other individuals on your wellness team by moving the template to a shared folder and/or by using the Share button in the top right corner of the Google Doc. If the Step 1 and Step 2 forms are completed in Google Docs, please convert to PDF Documents only, to avoid formatting issues. A Word version of each template is also available for anyone that is unable to access Google files.

The following sample wellness committee meeting agendas are designed as optional tools to support collaborative completion of the triennial assessment. These are not required components of the triennial assessment.
