Complaint Form

You must either print a blank complaint form and fill it out, or type in an English-only fillable complaint form and print it out.  The Complaint form, whether it is handwritten or typed, must be signed in front of a notary.

 

How to complete a Complaint Form

You must fill in all blanks on the form.  You must sign the form in front of a notary.  Your complaint will not be accepted without being notarized.

The Complainant is you.  The Respondent is the certificated teacher or school administrator you are making the complaint about.  If you are filing a Complaint against more than one Respondent, a separate Complaint must be filed for each one.

In Paragraph #3 of the Complaint Form, please list clear and concise facts that you believe violate professional conduct and ethics.  Attach additional sheets if necessary.

In Paragraph #4 of the Complaint Form, please list the full name, address, and telephone number of each witness to the alleged incident.

 

How to submit a Complaint

You can submit a Complaint by either:

  • Regular United States Mail to:
    Certification Investigations’ Office, P.O. Box 94933, Lincoln, NE 68509.
  • Email the completed Complaint Form to: nde.investigations@nebraska.gov

Complaints typed directly into the body of the email will not be accepted.
