Eligibility Check (SIY)

Used to request a review of a student record to determine whether the student meets established eligibility criteria for a program, service, or process. The submission should produce a Jitbit ticket categorized as Eligibility Check. Request Finish Strong eligibility information for a student. To be eligible for the Finish Strong graduation program, a student must have been under the jurisdiction of the juvenile court and have had at least one out of home placement while in high school.

The submission should produce a Jitbit ticket categorized as Credit Assistance. The credit assistance form can be used to request support related to student credits. This includes credit granted by NDE, partial credit, credit evaluations, and course credits in relation to the Finish Strong graduation requirements.

Transcript Request Form (SIY)

Used to request a student transcript or transcript-related review. The submission should produce a Jitbit ticket categorized as Transcript Request. Use this form to request a transcript from NDE. This form should only be used if a student has either been granted credit by NDE or has been graduated by NDE on the Finish Strong graduation plan. Students who graduated from a public high school using a 200-credit minimum graduation plan need to contact their public high school.
