Application for child care centers

Initial Application - This set of forms is for first-time centers/sponsors. Use this set of forms if your center/sponsoring organization does not have a current CACFP agreement with the Nebraska Department of Education.

If you are a home care provider, you may not participate on the CACFP directly through the Nebraska Department of Education. You must participate with one of Nebraska's approved family day care home sponsoring organizations. more information
