EDUCATOR PREPARATION ANNUAL BOARD APPROVAL
NDE 20-004 REPORT

Institutions are required, on an annual basis, to submit NDE Form 20-004, Application for Approval for Specific Programs of Educator Preparation. In addition, institutions are responsible for including Rule 24 matrices for all new and revised endorsement areas to their Educator Preparation Program with the 20-004 form. The files listed below contain the matrices.

You will not have to re-do any draft versions you complete prior to their approval unless there are changes made during the approval process.
