EETP LOAN RECIPIENT FORMS
To meet the requirements of the Enhancing Excellence in Teaching Program (EETP), loan recipients must complete and submit the following forms, along with the required supporting documentation, to the Nebraska Department of Education (NDE). Failure to comply may result in the denial of future loan requests under this program and/or the commencement of repayment requirements

Proof of Registration 
Prior to the disbursement of EETP loan funds for eligible coursework, you are required to provide the Nebraska Department of Education (NDE) with proof of registration for the course(s) for which you are requesting payment. A semester/term schedule or transcript that provides the course code, course title, start/end dates and applicable credit hours is considered acceptable proof of registration.
To request payment for eligible coursework, the Proof of Registration Form must be completed in its entirety and submitted with the required supporting documentation to NDE. Upon receipt, NDE will process a payment in an amount of up to $175 for each applicable credit hour reported on the proof of registration. Please allow 2-4 weeks for payment processing.
You have up to thirty (30) calendar days from the end of the loan award period for which you received the loan to request payment for eligible coursework. Submissions received after the deadline will not be processed, and any remaining funds will be returned to the EETP Fund.
Proof of Coursework Completion
An EETP loan recipient who received payment(s) for eligible coursework is required to provide NDE with proof of completion of the coursework for which EETP funds were paid. A copy of a grade report or transcript is considered acceptable proof of completion.
The Proof of Coursework Completion Form must be completed in its entirety and submitted with the required supporting documentation to NDE. This may be done after each session or semester or as a composite for the award loan period.
Proof of completion for all courses for which you received payment must be submitted to NDE within sixty (60) days from the end of the loan award period.
Proof of Program Completion
To comply with the conditions of the EETP Contract, upon completion of the graduate program for which you received an EETP loan, you are required to provide NDE with proof of program completion in the form of an official transcript.
The Proof of Program Completion Form must be completed in its entirety and submitted with the required supporting documentation to NDE within sixty (60) days from the date you completed the graduate program.
