Special Curriculum Forms

Special forms for nonpublic schools are filed, if necessary, when reporting curriculum through NSSRS

  • Biennial Courses Form (NDE 08-023) - There are two forms available for your use, the second one allows you to fill in the form on your computer before printing. Otherwise, they are the same form.

  • Cooperative Arrangements Course Form (NDE 08-021) - There are two forms available for your use, the second one allows you to fill in the form on your computer before printing. Otherwise, they are the same form.

School Closure Due to Uncontrollable Circumstances

If schools are unable to meet the 450/400/1032/1080 instructional hours, the following affidavit may be filed 10 days after the end of the school year
